If you are purchasing the Annual License Fee on behalf of someone else, OR you are purchasing more than 1 Annual License Fee, please ensure that the name of the Instructor(s) you are buying for is typed out in the Order notes section of the checkout.
To access the Order notes panel, scroll down to the bottom of the screen in the checkout section of the MHFA Shop.
*Note: You do not have to tick the "Are you purchasing for someone else?" checkbox for this purchase.
Please ensure the number of Annual License Fees matches the number of names you add in this section.
We will use these names to ensure your purchase is credited to the Instructor's membership.
